What Is the Omega-3 Index?

The Omega-3 Index measures EPA (eicosapentaenoic acid) and DHA (docosahexaenoic acid) as a percentage of total red blood cell fatty acids. Because red blood cells turn over approximately every 120 days, the Omega-3 Index reflects long-term omega-3 status — similar to how HbA1c reflects long-term glucose.

An Omega-3 Index below 4% is considered high risk. The optimal range is above 8%. Most adults in the United States have an index between 4–6% — below the optimal threshold.

EPA vs. DHA — Different Roles

EPA and DHA are both omega-3 fatty acids but serve different primary functions. EPA is more anti-inflammatory and plays a larger role in cardiovascular protection. DHA is the dominant structural fatty acid in the brain and retina, and is critical for cognitive function and neurological health.

Both are important. Fish oil supplements typically provide both. Algae-based supplements (the original source of DHA in fish) are an effective alternative for those who avoid fish products.

Cardiovascular Benefits

High-dose EPA supplementation (specifically icosapentaenoic acid at 4g/day) has been shown in the REDUCE-IT trial to significantly reduce major cardiovascular events in high-risk individuals. The cardiovascular benefits of omega-3s appear to be dose-dependent — meaning higher blood levels produce greater protection.

Omega-3s reduce triglycerides, improve endothelial function, reduce platelet aggregation, and have anti-inflammatory effects on arterial walls. They are one of the few supplements with robust cardiovascular outcome data.

The Omega-6 to Omega-3 Ratio

Modern diets are heavily skewed toward omega-6 fatty acids — primarily from seed oils (soybean, corn, sunflower, canola) used in processed foods and restaurant cooking. The omega-6 to omega-3 ratio in ancestral diets was approximately 4:1. In modern Western diets, it is often 15:1 to 20:1.

This imbalance promotes a pro-inflammatory state. Improving the ratio requires both increasing omega-3 intake and reducing omega-6 consumption from seed oils.

Supplementation Considerations

Not all fish oil supplements are equivalent. Oxidized fish oil may be harmful rather than beneficial. Key quality indicators include: triglyceride form (more bioavailable than ethyl ester), third-party testing for oxidation (TOTOX score), and freshness (smell and taste).

Typical supplementation doses range from 1–4g of combined EPA+DHA daily. Higher doses are used therapeutically for triglyceride reduction and in high-risk cardiovascular patients. Testing your Omega-3 Index before and after supplementation is the most reliable way to confirm adequacy.
